 석면폐증의 병형이 진행형 또는 초기형이고 폐기능 장해단계가 고도장해인 경우 |

※피해인정신청 시 필요한 진단서 및 검사서는 법 제44조 제1항 및 이 영 제 40조에 따른 의료기관에서 발급한 진단서에 한 한다
(단, 법시행전 이미 의료법 제3조에 따른 의료기관에서 석면 질병에 대한 진단 및 검사를 받은 경우에는 그 서류를 신청서류로
